  • 3 Month Minimum and then month to month, no contracts
  • Exclusivity Policy (15-mile radius-city, 30-mile radius- rural)
  • Minimum 50 New Patient Opportunities Every Month GUARANTEED or We Will Waive Ads Management FEE Until We Hit the Minimum
  • We Need 100 Percent Control of the Offer and Ad Creative To Ensure Consistent Results and Avoid Ad Fatigue

  1. Complete all trainings and onboarding calls within 10-14 days
  2. *Get us photo and video assets within 10 days.
  3. *Accurately use and update your personal CRM / UTM stats & pipeline.
  4. *Make a total commitment to follow our system
  5. We need 100% visibility to revenue generated from our services in real-time to measure the success of our partnership month to month
  6. Return on Investment must be calculated over a 3 month period NOT any one month to accurately calculate secured revenue generated
  7. $1500 / month minimum ad spend is required for 50 New Patient Opportunity guarantee

grant permission to Web Profit Maximiser, hereinafter known as the “Media” to use my image (photographs and/or video) for use in Media publications including the following:


- Videos - Email Blasts - Recruiting Brochures - Newsletters - Magazines - General Publications - Website and/or Affiliates - Facebook Advertising


I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.

I am 20 years of age or older and I am competent to contract in my own name. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release.
